Except for the basic documents (passport, photo and filled application form), you have to provide her an invitation letter, both original and copy of your resident permit and passport and the kinship certificate (marriage certificate for example).

I am not sure if she can convert it successfully without leaving mainland China. Someone says that it's impossible to do it. If she can't, then she can leave for HK and apply for it there.
