Chengdu to Kathmandu via Lhasa

I am travelling in China during October, starting in Beijing and hoping to end the trip by entering into Nepal (Kathmandu ) from Tibet overland.

Can anyone tell me the best way to do this and/or recommend a travel agent that will sort the permits etc for tibet?

I am a Brit BTW.
