Strange request, need help please.

I have a very unusual allergy , it causes me problems and may one day kill me. I am allergic to alcohol, any type of alcohol. I can not drink any and I cant eat any food that has been cooked using alcohol or marinaded. Last year I ate chicken cooked with wine and I was in hospital staright after and was lucky to survive.
I was wondering if someone could write something in Chinese stating that I have a problem and any food I eat must be free of alcohol so I can print it and carry it around when I go to Chinese food stalls or restaurants.

You may say, "我有严重的酒精过敏，菜里千万别放酒，料酒也不行．＂(I have a serious allergy to alcohol. Don't put any wine in food. Rice wine is not available as well).

"料酒＂（liao 4 jiu 3) is a kind of rice wine which Chinese usually use to cook fish or meat.

That would include beer too wouldn't it? What about vinigars etc??

it is just any form of alcohol. vinegars are generally ok
