Hi there. I have been to China in the past, and have always used a 2-page application form for my visa...I am Canadian citizen in Calgary. I now see that the Ottawa Chinese Consulate website has an application form that is 4 pages long, while the Calgary Consulate still offers visa applications that are 2 pages long. Does anyone know whether this is a new form that must be used? I do not want to apply in Calgary only to have filled out incorrect applications! (especially since I live 3 hours away!)Thanks!

The visa application form with 4 pages is the new form, but the old form with 2 pages is valid until June 30th, 2011. After June 30th, 2011, you have to use the new form with 4 pages.

For other readers, I believe there are new forms across many (all?) countries later this year. One hopefully useful change is there is (in the UK at least, but I assume elsewhere) a second form for persons applying outside the country of their passport.
