Where to get 'proof of guardianship' legalized?

guest12162 Hey, I've got a visa problem for a young child that requires 'proof of guardianship', legalized by the MFA.

The question is: where can I get the form necessary, and where can I get it MFA legalized?

GUEST40882 You can get the application form at the official website of the embassy of PRC.
After preparing all the required documents, you should get the guardianship document notarized by the local notary public and then get the document legalized by the ministry of foreign affairs (MFA) of your home country.
