When we meet someone whom you don't know, we will greet him/her at first. Then we can introduce ourselves. We can say 我的名字是张三 (Wo 3 De 1 Ming 2 Zi 4 Shi 4 Zhang 1 San 1) or more commonly 我叫张三 (Wo 3 Jiao 4 Zhang 1 San 1).

我 means I, Me and 我的 means My. We can also say 你的 (Ni 3 De 1), 他的(Ta 1 De 1),她的(Ta 1 De 1), which means Yours, His and Hers. 名字 equals to Name in English and 是 equals to Is. So 我的名字是张三 means My name is Zhang San.

我叫张三 is a more common way to say My name is Zhang San. 叫 is a verb here, equals to Call. So 我叫张三 can be translated into I'm (called) Zhang San or My name is (called) Zhang San.

e.g.:
Sam: 你好!
May: 你好!
Sam: 我的名字是山姆(Shan 1 Mu 3, the Chinese translation of Sam)。
May: 我叫梅(Mei 2, the Chinese translation of May)。

It might be quite common that expats are asked "Where do you come from" or "What's your nationality". How to answer these questions in Chinese? Chinese may ask 你是哪国人?(Ni 3 Shi 4 Na 3 Guo 2 Ren 2) which means What's your nationality. 哪 means Which here, 国 means Country, Nationality, State, 人 means People. So literally 你是哪国人 means Which country's people are you? Then you may answer 我是英国人 (Wo 3 Shi 4 Ying 1 Guo 2 Ren 2) <means I'm British, just for example.>

Sam:我是美国人(Wo 3 Shi 4 Mei 3 Guo 2 Ren 2, means I'm American)。
