Most people raise doggies as their pets but few people raise dogs for other purposes. Look at this old man. His doggie pulls a small cart.

He seems very tired and thirsty.

GUEST12982 Maybe the dog is a little thirsty and tired, but the cart is on wheels and it doesn't seem very heavy.
I think the man is using this as an eye catcher, perhaps as a subject to start a conversation, or a "pick up" tool (people have used dogs/pets to find mates forever...).

Why not let a Tibetan Mastiff do this job? He is stronger. And I am sure that he will be known to everyone.

The dog seems to be just fine. It is doing what it thinks is its job, helping his master, the leader of the pack. And of course, if it gets some positive feedback from his master, it will be as happy as a dog can be. In addition to that, a little training does not do any harm for people nor for dogs...
